An anal fissure (fissure-in-ano) is a small tear in skin that lines the opening of the anus. Anal tears (fissures) are common and affect about 1 in 350 people each year. They are more common in people aged between 15 and 40 years but can occur at any age, including in very young children. These fissures are also more common during pregnancy and childbirth. An anal fissure occurs in about 1 in 10 women during childbirth. But, they are less common in the elderly.
Fissures cause severe pain around the anus and are likely to develop with Constipation. The pain can be really bad and tends to be worse when you pass stools (faeces) and for an hour or so after passing stools. Stretching and tearing can occur when a person passes particularly hard stools. Consequently, you may also get some bleeding when you pass stools, and hence are often confused with other causes of pain and bleeding, such as haemorrhoids. Sometimes the tear occurs also if you have bad diarrhoea.
Spasm of the internal sphincter has also been cited to cause fissure in-ano. This apart, when too much skin has been removed during operation for haemorrhoids, anal stenosis may result in which anal fissure may develop when hard motion passes through such stricture. 3. Secondary causes of anal fissures are Ulcerative colitis, Crohn’s disease, Syphilis and Tuberculosis.
